Community Options
What is Community Options?
Community Options is a service that supports the residents of the Queanbeyan region stay in their own homes by connecting them to a range of support services.
Community Options can provide:
- a comprehensive assessment;
- coordinated care planning;
- flexible services for people with complex needs;
- advice and information.
Who can benefit from Community Options?
The service can support people who live in the Queanbeyan Local Government Area and are:
- older people who are frail;
- people with moderate to sever disabilities; and
- carers of these people.
When should you contact Community Options?
Contact us is you are:
- unable to cope at home and need help with day to day tasks;
- unable to obtain the services or equipment that you need;
- needing a variety of services co-ordinated to meet your individual needs;
- looking for information about available community care and support services;
- require information about residential care facilities;
- a carer and require support to maintain your role in the community.
How the Community Options Program can help
Community Options can help by:
- visiting you at home;
- listening to you if you are having difficulty at home;
- explaining the options available to help you and your carer at home;
- arranging support from a range of community services such as Home Care Service, Meals on Wheels and Respite Care;
- offering services that are flexible and not provided by other agencies (brokerage);
- provide referral pathways;
- supporting you if you decide to move to residential care.
Contacting Community Options
Anyone can make a referral to Community Options:
- doctor or community nurse
- relative or friend
- other service providers
- or you can refer yourself
Community Options may be able to help you or someone that you know.
Information that you provide remains confidential.
The Cost of Community Options
As a client, you will be encouraged to contribute to the cost of the services or equipment that you receive.
However Community Options can subsidise the charges if necessary.
No one will be refused basic assistance because they are unable to pay.
